Review Forms

TDA has defined a series of reviews forms for SFSP reviews.  The following describes the general process of completing a review form.

Completing a Review Form

Forms are organized by Contracting Entity and by Site.  The forms list and contents are defined by TDA.  All forms associated with a specific review and Contracting Entity/Site are listed in the left frame of the screen.  To access a specific form, select the desired form by highlighting it in the left frame.  The screen will display the form.

Note: A question with a red asterisk next to the question number is required and must be answered.

Identifying a Finding

The majority of review information is captured within individual Contracting Entity and site-level review forms. Typically, the Reviewer selects the radio button to answer each applicable question. Selecting a “red” answer triggers a finding. Selecting a “blue” answer triggers technical assistance. Selecting the  icon beside the question number enables the user to view or modify finding/technical assistance information.  The user can save the data at any time by selecting Save in the top-left corner.

Note: The  icon only displays when a red/blue answer is selected

Note: All finding logged on a specific form/question will automatically appear on the Findings/CAD screen once saved.

Entering an Comment/Note

The Reviewer can add notes to a specific question as a reminder of their exact finding.  The notes are not part of the finding, but can be used by the Reviewer to develop their formal finding.  Notes can be entered for all questions, regardless of the answer.  Notes are identified by the comment icon  to the right of the answers.

Completing an Embedded Worksheet

Some forms include embedded worksheets for the Reviewer to complete.  The worksheet resembles a Microsoft Excel spreadsheet.  Data can be entered either directly into the cell.  The user can use their keyboard’s arrow or tab keys to move throughout the worksheet.

If the worksheet results in a calculation (e.g., summation of data entered), the user would enter the information into the worksheet and then select the Calculate Worksheets button at the top of the screen.

Note: Data can only be entered into authorized designated cells. Attempting to enter data into a non-designated cell will result in an error.

Refreshing Reference Data

The laptop software uses a variety of reference data that is maintained on the TX-UNPS web system.  This includes Review Tracking information (e.g., who is scheduled for a review), Profile information (e.g., Type of Agency), Application information (e.g., site type), etc.  The data is copied onto the laptop when a download has occurred.  In order to refresh this data, click the Refresh Reference Data button at the top of the screen

TIP: The Download Reference Data action should be selected primarily just before a review is about to occur.  Generally, you should not select this button during a review because some reference data may have changed since you started the review (i.e., a CE could have modified their application). 

Make sure all data is saved before you click Refresh Reference Data.
